ABOUT THE UNIT

The Community Outreach Research Unit collaboratively develops intervention programme to promote early detection and improve access to breast cancer care for women seeking treatment at Ministry of Health hospitals. The Community unit together with other research units in CRMY intends to close the gap in cancer care and our current focus is on breast cancer. The reality today is that who you are and where you live could mean the difference in cancer survival. It isn’t fair. But we can change this by developing the i) most effective healthcare system changes for improving survival and ii) innovative and cost-effective tools for enabling early detection of cancer in B40 communities.

 

SCOPE / PURPOSE OF JOB

The community navigator promotes timely diagnosis and treatment and aims to ensure seamless, coordinated care and services. The community navigator helps patients and their families access the health care delivery system through support, education, communication, and resources. This support begins at the point of a suspicious finding and continues throughout treatment to the resolution of the finding. The navigator works with breast cancer patients to improve their cancer care outcomes.


RESPONSIBILITIES

  • Time management and keeping up with the dateline as per required with CRMY and MOH. Prioritization of task and activity carried out according urgency and need of breast cancer patients.
  • Provide navigation support for breast cancer patients who are diagnosed with breast cancer. Uses the patient navigation intake assessment and identifies needs and barriers that prevent patient from accessing care. Tracks patient progress and keeps other care team members informed. Identifies potential bottlenecks and performs appropriate interventions.
  • Assists patients across healthcare disciplines to ensure appointments are made and kept. Works closely with appropriate staff throughout the healthcare system to improve patient care outcomes
  • Tracks and follows patient interactions via appropriate documentation policies and procedures
  • Engages in program evaluation efforts and submits reports as necessary
  • Identifies system and/or organizational barriers to care and recommends strategies to improve access
  • Pays utmost attention to patient confidentiality; in order to facilitate care, patient navigator will have access to patient information including diagnosis, treatment plan, and personal information such as address to facilitate local community support and untapped services
  • Resource Identification and Utilization – Coordinating services with nurse navigator to assist patients with their basic needs, ensuring that they complete their diagnosis and treatment in a timely manner. This includes advising on entitlements and helping with completion of application forms, referring patients to state/national agencies or non-profit organizations depending on the needs of the patient. Plan and conducting home visits for i) welfare assessment, ii) patient or caregiver education, and iii) defaulter tracking.
  • Collaborates with local individuals, agencies, and organizations to facilitate access to community-based services addressing patients’ barriers to care.
  • Well versed with local resources and able to build a wider and sustainable network of community partner to address barrier faced by breast cancer patients.


Essential Requirements:


• Education

  • Diploma/ bachelor's degree in Nursing Science is a must, however if candidate possess Advance Diploma in Oncology Nursing or Palliative Care Nursing will be an added advantage.
  • Minimum of one to two year of experience in hospitality, social work, psychology, population-based services, home care, community officer, human activist, social entrepreneurship, social services, customer care and service, or any related experience or combination of education and experience that is similarly, equivalent.
  • Fresh graduates are encouraged to apply

• Full driving license and own a car/motorcycle

• Knowledge/Skills/Abilities

  • Effective professional communication skill, both orally and in written form ie, good letter writing skill.
  • Good computer skills (Microsoft Office and Virtual meeting platform)
  • Fluent in communicating in local languages: Bahasa Malaysia, Mandarin or, Tamil, Local Sabah and Sarawak languages is a strength.
  • Knowledge on the local population, resources and cultural sensitivity

BERKENAAN UNIT

Unit Penyelidikan Komuniti Outreach secara kolaboratif membangunkan program intervensi untuk menggalakkan pengesanan awal dan meningkatkan akses kepada penjagaan kanser payudara untuk wanita yang mendapatkan rawatan di hospital Kementerian Kesihatan Malaysia(KKM). Unit Komuniti bersama-sama dengan unit penyelidikan lain di CRMY berhasrat untuk mengurangkan jurang dalam penjagaan kanser dan tumpuan semasa kami adalah pada kanser payudara. Realiti hari ini ialah siapa anda dan di mana anda tinggal boleh memberi impak perbezaan dalam survival kanser. Ia tidak adil. Tetapi kita mampu mengubahnya dengan membangunkan i) perubahan sistem penjagaan kesihatan yang paling berkesan untuk meningkatkan kelangsungan hidup dan ii) alatan yang inovatif dan kos efektif untuk membolehkan pengesanan awal kanser dalam komuniti B40.

 

SKOP / TUJUAN PEKERJAAN

Komuniti Navigator pesakit payudara mempromosikan diagnosis dan rawatan tepat pada masanya, dimana ianya bertujuan untuk memastikan penjagaan dan perkhidmatan yang lancar dan terkoordinasi. Komuniti Navigator pesakit payudara juga membantu pesakit dan keluarga mereka mengakses sistem penjagaan kesihatan melalui sokongan, pendidikan, komunikasi dan sumber kesihatan. Sokongan ini bermula pada penemuan permasaalahan dan berterusan sepanjang rawatan sehinggalah kepada penyelesaian permasaalahan. Navigator bekerja dengan pesakit kanser payudara untuk meningkatkan tahap penjagaan kanser mereka.


TANGGUNGJAWAB

  • Pengurusan masa dan mengikuti garis masa seperti yang ditetapkan oleh CRMY dan KKM. Keutamaan tugas dan aktiviti yang dijalankan mengikut kesegeraan dan keperluan pesakit kanser payudara.
  • Menyediakan sokongan navigasi untuk pesakit kanser payudara yang telah didiagnosis sebagai kanser payudara. Menggunakan penilaian navigasi pesakit dan mengenal pasti keperluan dan halangan yang menghalang pesakit daripada mengakses penjagaan fasiliti kesihatan. Menjejaki kemajuan pesakit dan memastikan ahli pasukan penjagaan lain dimaklumkan. Mengenal pasti potensi gangguan yang mungkin berlaku dan melakukan intervensi yang sesuai.
  • Membantu pesakit merentasi disiplin penjagaan kesihatan bagi memastikan janji temu dibuat dan disimpan. Bekerja secara rapat dengan kakitangan yang berkenaan di sistem penjagaan kesihatan untuk meningkatkan hasil penjagaan pesakit.
  • Menjejaki dan mengikuti perkembangan pesakit berdasarkan dasar dan prosedur dokumentasi yang sesuai
  • Melibatkan diri dalam penilaian program dan menyerahkan laporan seperti yang diperlukan. 
  • Mengenal pasti halangan sistem dan/atau organisasi untuk menjaga dan mencadangkan strategi untuk meningkatkan akses.
  • Memberi perhatian sepenuhnya kepada kerahsiaan pesakit; bagi memudahkan penjagaan, navigator akan mempunyai akses kepada maklumat pesakit termasuk diagnosis, pelan rawatan dan maklumat peribadi seperti alamat untuk memudahkan sokongan komuniti setempat dan perkhidmatan yang belum pernah diberi. 
  • Pengenalpastian dan Penggunaan Sumber – Menyelaras perkhidmatan bersama jururawat navigasi untuk membantu pesakit berkaitan keperluan asas mereka, memastikan mereka menyelesaikan proses rawatan mereka tepat pada masanya. Ini termasuklah memberi nasihat tentang kelayakan dan membantu pesakit melengkapkan borang permohonan, merujuk pesakit kepada agensi negeri/negara atau organisasi tanpa keuntunga; bergantung kepada keperluan pesakit. Merancang dan menjalankan lawatan ke rumah untuk i) penilaian kebajikan, ii) pendidikan pesakit atau penjaga, dan iii) penjejakan pesakit yang keciciran.
  • Bekerjasama dengan individu, agensi dan organisasi tempatan untuk memudahkan akses kepada perkhidmatan berasaskan komuniti yang menangani halangan pesakit terhadap penjagaan kesihatan
  • Mahir dengan sumber tempatan dan mampu membina rangkaian rakan komuniti yang lebih luas dan mampan untuk menangani halangan yang dihadapi oleh pesakit kanser payudara.


KEPERLUAN PENTING:


• Pendidikan

  • Diploma/Ijazah Sarjana Muda dalam Sains Kejururawatan adalah satu keperluan, namun akan menjadi satu kelebihan jika calon memiliki Diploma Lanjutan dalam Kejururawatan Onkologi atau Kejururawatan Penjagaan Palliatif. Atau sebarang latar belakang berkaitan lain.
  • Sekurang-kurangnya satu hingga dua tahun pengalaman dalam bidang hospitaliti, kerja sosial, kerja sosial perubatan, psikologi, perkhidmatan berasaskan komuniti, penjagaan fasiliti kebajikan, pegawai komuniti, aktivis kemanusiaan, keusahawanan sosial, perkhidmatan sosial, penjagaan pelanggan dan perkhidmatan, ATAU gabungan pendidikan dan pengalaman yang serupa atau setara.
  • Graduan baru digalakkan untuk memohon.

Lesen memandu penuh dan memiliki kereta/motosikal. 

• Pengetahuan/Kemahiran/Kebolehan

  • Kemahiran komputer yang baik (Microsoft Office dan platform mesyuarat secara maya)
  • Fasih berkomunikasi dalam bahasa tempatan: Bahasa Malaysia, Mandarin atau, Tamil, Bahasa tempatan Sabah dan Sarawak adalah kelebihan.

We are an independent and non-profit cancer research organisation based in Malaysia. Funded by donations and research grants, we conduct research in niche cancers often found in our Asian population.
